All members are advised that Caution Reports, Send-Off Reports and Incident Reports must be sent to the C.&.D.S.F.A. Judiciary Committee no later than Tuesday night after the game. It is advised that all reports are submitted by Tuesday night no later than 6.00pm. Copies of Incident Reports & Send Off Reports only must also be sent to our Secretary preferably the same night as the incident.

Caution Report Forms and Send-Off Report Forms may be collected at our monthly general meetings or may be printed from here (In MS-Excel format) :
