MATCH-DAY CLUB INCIDENT FORM
To all,
Attached is a generic “match day” incident report form you can use at your home grounds.
Incident Form - Match Day (Club)
It can be copied and left in an accessible place at your Club (clubhouse / canteen) for people to use to advise the Club of any incidents that may occur. This is a first step form that can be filled out at the time and left at the park on the day to be collected by a Club representative. The Club can then follow up – if required – the incident internally or externally.
NOTE: Incidents are not restricted to just violence during or after games. It can be anything from change-room damage, calling of an ambulance, unauthorised persons within the fence/rope, etc.

REMINDER – CLUBS / TEAM OFFICIALS / GROUND OFFICIALS:
ALL CLUBS are to remind their Team Staff and Ground Officials of rule 8.3 outlining who is permitted within the ropes/fence on a football field. The NO CARD / NO ENTRY policy must be enforced strongly by all Clubs. Reported failures to comply with this rule to the Association may require the Club concerned to face the CDSFA Executive Committee for a “please explain”.
I urge Clubs to ensure they have ID Cards printed for all their Coaches, Managers and Officials prior to the season commencing.
